An Authorization for Medication to be Dispensed by School Personnel form is required for each medication your student may need to take on this field trip including all prescribed and over the counter medications required for retreats and field trips.

If the student MUST carry a non-controlled, rescue medication, the student's physician and parent must make this determination on the authorization form. This revocable privilege will also need to be authorized by the principal and health office, who will issue a QR Coded sticker and assigned number.

YOUR STUDENT WILL NOT BE ABLE TO ATTEND THIS FIELD TRIP WITHOUT THIS FORM AND APPROVED AUTHORIZATION FROM OUR HEALTH OFFICE.
